Rodem Community Care Pty Ltd provides professional, person-centred community nursing and disability support services for NDIS participants, families and care teams across Australia.

Our goal is to deliver safe, respectful and practical nursing care that supports health, dignity, independence and quality of life. We work with participants, nominees, families, support coordinators, plan managers and service providers to ensure care is clear, coordinated and clinically appropriate.

Our Approach

We provide care that is practical, respectful and easy to understand. Our assessments and recommendations are tailored to each participant’s health needs, daily routine, support environment and NDIS goals.

Where appropriate, we liaise with GPs, specialists, allied health professionals and care teams to support safe and consistent care.
